Insurance
As a rule, Dixie Family Skills Center does not take insurance. However, we are capable of completing HC1500 forms if you are seeking reimbursement from your Insurance Provider. In order to know whether your insurance carrier will cover out-of-network mental health services, it is recommended that you contact them to obtain that information. It is expected that services will be paid for prior to session. HC1500 Forms will be provided after 6 sessions, unless services are terminated prior.
Payment
Cash, check and all major credit cards accepted for payment.
Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have
not notified us at least 24 hours in advance, you will be required to pay
the full cost of the session.
